Care After Hair Transplant

To achieve optimal hair follicle growth after a hair transplant, it is essential to follow proper care. Most of the important care lasts only for the first 7 days. This section provides a brief overview of how you should maintain the transplanted area during this period.

The basic principles include:

  • Caring for the recipient area (the planted grafts)
  • Caring for the donor area (the area from where grafts were obtained)

These steps are straightforward and require only about 7 days. Most patients follow these guidelines comfortably without difficulty.


Management of the Recipient Area After Hair Transplant

Using Sterile Saline or RL (Ringer’s Lactate Solution)

You should spray sterile saline or RL every 30 minutes for the first 2 days. This keeps the area moist and prevents excessive dryness. Dryness can cause itching and lead to adherent scabs, which makes cleaning difficult. Keeping grafts moist during the first 1–2 days also improves graft survival.

Using Shaving Foam

Use shaving foam as a mild moisturising cleanser for the recipient site. Choose any routine non-perfumed shaving foam. Apply it three times daily for the first 7 days to maintain cleanliness and hydration.

Performing a Head Bath

Start a head bath on the 7th day after the procedure. By this time, grafts adhere more firmly to the skin. Although grafts will not easily dislodge, you must still wash gently.


Management of the Donor Area

Dressing the Donor Area

The doctor covers the donor area with a bandage and cotton pads immediately after the procedure. The doctor removes this dressing the next day at the clinic. After removal, you do not need further bandaging. Mild soaking or oozing for a day or two is normal.

Application of Antibiotic Ointment

Apply prescribed antibiotic ointment such as T-Bact cream or Neosporin Skin Ointment to the donor area three times daily starting from the second day. Continue application until day 7.

Care of Scabs in the Donor Area

Small scabs usually appear after 3–4 days. Allow scabs to fall off naturally. Do not remove them manually. The donor area generally heals completely within 7 days.

Washing the Donor Area

Avoid head baths during the first 6 days. Clean the donor area gently with a wet cloth during this period. Avoid soap for the first 2 days while scabs form. After day 2, you may use shampoo gently.


Important Issues You Must Know After Your Hair Transplant

Pain

Most patients experience minimal or no pain after transplantation. Doctors usually prescribe painkillers such as Combiflam or Emanzen-D. If pain occurs, it typically happens when touching the donor area and is completely normal.

Lost Grafts

You may lose a few grafts after the procedure. This is expected and should not cause concern.

Swelling

Swelling may appear between the 2nd and 5th day after surgery. It usually begins near the hairline and gradually moves downward due to gravity, sometimes reaching the eyelids. The swelling resolves naturally without treatment.

To reduce swelling:

  • Apply cool compresses (avoid direct pressure on grafts)
  • Sleep with your head elevated at a 45-degree angle for the first 3 nights
  • Use a recliner chair or 3–4 pillows

Contact the clinic if swelling occurs with redness, pain, fever, tenderness, or chills, as these may indicate infection.


Activity, Alcohol and Other Restrictions

Take rest the day after surgery and focus on post-operative care. You may resume normal daily activities and mild exercise from the second day. Avoid heavy lifting and activities that cause sweating for the first 10 days.

  • Avoid alcohol for at least 7 days
  • Avoid rubbing or putting pressure on the scalp for 10 days
  • Resume regular exercise after 10 days

Smoking

Smoking reduces blood flow to the scalp due to nicotine-induced vasoconstriction and lowers oxygen delivery because of carbon monoxide exposure. These effects delay wound healing, increase infection risk, and may affect hair growth.

Patients should ideally avoid smoking from 1 week before surgery until at least 2 weeks after the procedure.


Itching Over the Recipient Area

Itching commonly occurs in both donor and recipient areas during healing. Avoid scratching, as this may dislodge grafts. If itching becomes bothersome, gently tap or touch the area with your fingertips instead of scratching. Proper saline spraying and shaving foam use usually minimize itching.


Sun Exposure

Avoid direct unprotected sunlight for 1 month. Wear a cap outdoors. After two weeks, you may apply sunscreen with SPF 30+.


Hair Colouring

You may start hair colouring or dye application one month after transplantation.


Numbness

Temporary numbness or tingling may occur due to minor nerve disruption during graft harvesting and site creation. Sensation usually returns gradually over weeks to months as nerve endings regenerate.
